Telehealth Services & Pricing

Cash-pay telehealth services are currently available only to patients who are physically located in

Indiana, Illinois, Iowa, Colorado, and Florida in accordance with state licensure requirements.

The prices listed on this website reflect consultation and medical service fees, costs of prescribed compounded weight management medications are also listed separately below for price visibility. Prices do not include the cost of recommended supplements or laboratory testing fees when applicable.

All services are offered on a cash-pay basis only. Accepted payment methods include all major credit cards including HSA/FSA and Zelle payments. For same-day sicks visits medications will be sent to your local pharmacy of choice.

Insurance is not accepted for compounded medications or visit fees. The pricing listed on this website is subject to change without notice. The total cost of treatment may vary depending on the medication prescribed and dosage requirements.

If determined that FDA-approved brand medications are covered under the patient's insurance pharmacy benefits, patients will be required to contact their preferred pharmacy for pricing and medication availability. Standard visit fees are displayed for transparency. Because treatment plans and medication options vary from patient to patient, the total cost of care cannot be determined until your consultation is performed.

Pediatric Evaluation and Treatment Disclosure for Weight Loss Visits

WeightWise Virtual Clinic provides medical evaluation and treatment for weight management in adolescents when clinically appropriate. A parent or legal guardian must provide consent for care and participate in the telehealth visit.

WeightWise Virtual Clinic provides specialized weight management services and does not replace a child’s pediatrician or primary care provider. Pediatric patients must maintain an active relationship with their pediatrician for routine care and monitoring of growth and development.

GLP-1 medications, including both FDA-approved brand medications and compounded formulations, are not prescribed to patients under the age of 12 years. Adolescents being considered for medication therapy must generally meet clinical criteria for obesity, including a body mass index (BMI) at or above the 95th percentile for age and sex based on pediatric growth charts.

If it is determined that the patient qualifies for medication through their insurance weight loss benefits, a prescription for a non-compounded medication may be sent to their local pharmacy for processing and a prior authorization will be completed on the patient’s behalf without an additional charge.

Adult Patients

For adult patients, certain symptoms, conditions, or clinical findings may require referral to a higher level of care. This may include, but is not limited to:

  • Emergency departments for urgent or life-threatening conditions

  • In-person primary care or specialist evaluation

  • Diagnostic testing that cannot be performed virtually

  • Advanced imaging or procedures

If at any point your condition is determined to be outside the scope of safe telehealth management, you will be advised to seek in-person care or emergency services as appropriate.

Pediatric Patients

For pediatric patients, safety is especially important. While many minor conditions can be evaluated virtually, children may require in-person assessment more frequently due to:

  • Need for physical examination

  • Rapid changes in condition

  • Age-specific considerations

Parents or guardians will be advised when a child should be evaluated in person, including referral to:

I am Dr. Desiree Samples, DNP, RN, FNP-BC, a board-certified Family Nurse Practitioner with over 15 years of experience as a Nurse Practitioner. I have a passion for helping patients achieve sustainable, healthy weight loss through education, accountability, and compassionate care. I hold a Doctor of Nursing Practice degree with a designation as a Clinical Expert.

With over 21 years of experience across primary care, hospice and palliative medicine, inpatient hospital settings, behavioral health, retail health, and wellness settings, I have seen firsthand that lasting health transformation requires more than a prescription. It requires partnership. My approach combines clinical expertise with practical lifestyle strategies to help you build habits that support long term success.
